  1. Download this form, either fill out and sign digitally, or print, fill out, sign, and scan it to a file

  2. Submit this form, be sure to attach the signed reimbursement form and all the receipts

  3. Once approved, PTSA will write a check payable to you, notify you by email, and the check can be picked up from the school office
